The document comprises a three-page report and a final page with a blank sample "registry" sheet with a survey for gathering the required information on people diagnosed with AIDS, including whether they were a homosexual, as well as details regarding their drugs use. *PLEASE NOTE THIS IS A PHOTOCOPY* however, it does appear that it is a period, rather than later, photocopy.
